An initial consultation takes longer than a follow up visit as it takes time to get a good history, do an examination discuss the findings possible diagnoses discuss if any furthur imaging or blood tests are needed and develop an action plan. This actually saves time later on as a lot of questions can be answered and possible therapy alternatives discussed.
